    Job Summary

    Provides care and education on disease management to cardiac, pulmonary, and CHF patients in outpatient cardiopulmonary rehabilitation.  Provides care to inpatients and outpatients with complex wounds, ulcers, and ostomies.  May assist in providing education to patients with diabetes in the hospital setting. May also provide preventative care and skills to the surrounding community through wellness events and disease management programs.

    Job Responsibilities:

    1. Provides care to patients with cardiac and pulmonary disease in a rehabilitation and disease management program following AACVPR, ASCM and other regulatory body guidelines.
    2. Engages in case management of cardiac and pulmonary patients to assure effective outcomes and addresses the need for continued services.
    3. Provides treatment to patients with wounds and ostomies in outpatient and inpatient settings. 
    4. Coordinates discharge plan that is patient/caregiver/family centered and includes understanding of patient’s home physical environment, as well as physical and emotional needs.  Informs patient/family of community and hospital resources and product supply companies for wound and ostomy patients as needed.
    5. Assists with the Ostomy Support Group and other disease management or wellness programs for the community as needed. 
    6. Provides assistance for inpatient diabetes education and care management. 
    7. Demonstrates flexibility to meet department, organization and patient/family needs.  
    8. Supports patient’s rights and responsibilities, including respect of cultural, ethical and religious diversity.
    9. Provides age-appropriate and population specific patient care in a safe, skillful, accurate and efficient manner to adult and geriatric populations.
    10. Assesses patient and family learning needs and provides ongoing education and evaluation.  Documents readiness to learn, teaching, barriers and outcomes.
    11. Completes documentation accurately and in a timely manner, according to hospital policies and procedures.
    12. Effectively utilizes the nursing process to ensure optimal nursing care and patient outcomes.
    13. Engages in activities to expand and/or enhance the professional nursing role, which includes integration of research and evidence based practice.
    14. Provides leadership and autonomy in the professional practice setting and the profession. 

     

    Qualifications

    Education Requirements and Other Requirements:

     

    Education Level: Graduate of an accredited school of nursing; BSN preferred. 

                   

    Certification/Licensure: Current license with the State of Illinois required. BLS certification required.  ACLS certification preferred or within 6 months of hire.

     

    Experience Requirements: Minimum of 2 years Medical/Surgical Nursing required. Experience with cardiac nursing, critical care, diabetes education, and/or wound and ostomy patients preferred. 

                   

    Working Conditions: Exposure Category I

    Task involves exposure to blood, body fluids, or tissues.  All procedures or other job-related tasks that involve an inherent potential for mucous membrane or skin contact with blood, body fluids, or tissues, or a potential for spills or splashes of them, are Category I tasks.  Use of appropriate measures should be required for every employee engaged in Category I task.
